  • At Wales level, nearly one in four registered patients were recorded as having any mental health condition and one in seven registered patients were recorded as having any chronic condition.
  • The percentage of patients recorded on each disease register ranged from 16.3% for hypertension to 0.3% for palliative care at Wales level.
  • At the Wales level, there has been a steady upward trend over the last four years in the percentage of patients recorded on the obesity register. For all other disease registers, changes have been more modest over both the short term and the medium term.
  • Chronic conditions are most prevalent among the oldest patients and decline steadily with age, whereas any mental health conditions are more evenly distributed across age bands, peaking among those aged 35 to 54. For most individual disease registers, prevalence increases with age.
  • The estimated percentage of people suspected to have dementia who were diagnosed with it has increased steadily over the last four years at Wales level.
