Overview
The 3 year Learning Disability Transformation Programme will improve support for people with learning disabilities across Wales.
It will help people live healthy, independent lives in their communities and reduce the need for hospital care.
The programme supports the development of a 10 year Learning Disability Strategy and takes a whole-system to service change and improvement approach across health, social care and prevention services.
It responds to evidence that people with learning disabilities need better community support, earlier help and more joined-up services. It builds on learning from previous reviews and engagement with people with lived experience, families, carers and organisations across Wales.
The programme aims to improve outcomes, strengthen rights and ensure people receive the right support in the right place at the right time to fully meet their individual needs.
Workstreams
The programme is organised around 4 workstreams:
- data, information and advice
- models of integrated community care
- specialist services, including inpatient care and crisis support
- accommodation and housing solutions
These workstreams will:
- support prevention
- improve access to services
- reduce unnecessary hospital admissions
- help people receive support closer to home
They will also strengthen planning, partnership working and the use of data and evidence to improve services.
What the programme aims to deliver
The programme aims to deliver:
- high-quality information, advice and assistance
- good access, experience and outcomes in community health, social care and preventative services
- access to specialist health and social care support in the community 24 hours a day, 7 days a week
- purposeful therapeutic hospital admissions, high-quality care and timely discharge
- a skilled and experienced workforce
- a data and evidence-informed system
These outcomes will help create a more consistent, person-centred system that supports people with learning disabilities to live well and achieve their goals.
Programme management
The programme is managed and overseen by NHS Performance and Improvement in collaboration with the Welsh Government.
