Background
When you answer the survey, we will handle your personal data according to our official authority to undertake the core role and functions of the Welsh Government and we will retain your information in a secure environment and access to it will be restricted according to the ‘need to know’ principle.

We will collect the following information using Smart Survey.

- Organisation
Why do we need this information?
Welsh Government work in collaboration with the WLGA and partners on the Streamlining Welsh Benefits programme. The aim of this survey is to find out what each local authority is doing individually to progress the Streamlining Welsh Benefits work following the recommendations from the Phase One Progress report.
The outcomes from the survey will evidence which local authorities have achieved the “tell your story once” approach and can be utilised as best practice models and demonstrate which local authorities may need more support and guidance to successfully implement the tell us once approach. The review may also identify barriers within individual local authorities which may be different from those that were identified during the Phase One.
We may use your contact details to follow up on responses and arrange meetings to discuss specific barriers.
Who holds it?
The Welsh Government will keep your personal information. If we wish to use your personal data for any other reason other than to follow up on Streamlining Welsh Benefits work we will request your permission.
Do we use it for any other purpose?
We may use your information to arrange discussions with Welsh Government officials leading on the Streamlining Welsh Benefits programme on progress.
How long do we keep this information?
We will keep your personal information for no more than one year after the survey.
If we need to retain information for longer periods for any reason we will inform you. You can request that your information is deleted at any time, by contacting tacklingpovertyandsupportingfamilies@gov.wales mailbox.

Individual rights
Under UK GDPR, you have the following rights in relation to the personal information you provide as part of this survey, specifically you have the right:
- to access a copy of your own data
- for us to rectify inaccuracies in that data
- to object to or restrict processing (in certain circumstances)
- for your data to be ‘erased’ (in certain circumstances)
- to lodge a complaint with the Information Commissioner’s Office (ICO) who is our independent regulator for data protection
