Survey on new emergency and acute care policy framework: privacy notice

The Emergency Care Policy Team in Welsh Government is undertaking a survey to inform the development of a new policy framework for emergency and acute care, which will aim to improve outcomes and experiences for people who need emergency and acute care services in Wales. The aim of this survey is to seek the views and experiences of health and social care staff on how they feel the emergency and acute care system is performing, how it interacts with the wider health and care system, and how it could be improved.

We are particularly keen to hear from health and social care staff whose experience and expertise provide valuable insight into how the emergency and acute care system is currently performing and where improvements could be made. Your feedback will help shape future policy and ensure the framework reflects the experiences of those delivering and supporting care across Wales.

The Welsh Government is the data controller for the survey. Personal data will not be used in the published policy framework.

Personal data is defined under the UK General Data Protection Regulation (UK GDPR) as ‘any information relating to an identifiable person who can be directly or indirectly identified by reference to an identifier’.

You have been invited to take part in this survey either via;

• an intermediary, for example local health board communications leads who hold staff contact details so they can distribute important organisational communications to the right staff groups, including surveys; 
• contact details on the NHS Wales Directory of Services – staff directory; 
• securely held Welsh Government contact details for organisations and staff groups we regularly engage with e.g. local health board chief operating officers; or 
• publicly available contacts details e.g. through organisations’ own websites.

This survey does not require the collection of additional personal data from you.

Your participation is voluntary and if you do not wish to take part or be sent reminders then please email jamie.wardrop001@gov.wales and your details will be removed. The policy team will only use email addresses and telephone numbers for the purposes of this survey.

If you choose to provide additional personal data as part of the survey we will try not to identify you from, or link your identity to, the responses you provide. 

If you raise a query or complaint and provide personal data requesting a response, we will forward the request only to the relevant official and subsequently delete it from the survey data.

The lawful basis for processing information in this data collection exercise is our public task; that is, exercising our official authority to undertake the core role and functions of the Welsh Government.

Participation is completely voluntary. Surveys such as this are important for the Welsh Government to collect information and actionable evidence about its ability to deliver government priorities. The information collected in this survey, for example, will be used to help inform policy development. 

Personal data provided to Welsh Government is held on secure servers, and information will only be processed by Welsh Government officials and will not be shared. 

When conducting surveys, the Welsh Government use a survey software programme called SmartSurvey. We have ensured that SmartSurvey is UK GDPR compliant and meets our expectations in terms of the security of any data collected via the software.

The Welsh Government has procedures to deal with any suspected data security breaches. If a suspected breach occurs, the Welsh Government will notify you and any applicable regulator where we are legally required to do so.

Any personal data will be deleted by the policy team within 6 months of the publication Emergency and Acute Care Policy Framework. This includes your contact details.

Under UK GDPR, you have the following rights in relation to the personal information you provide as part of this survey specifically you have the right:

• To access a copy of your own data; 
• For us to rectify inaccuracies in that data;
• To object to or restrict processing (in certain circumstances);
• For your data to be ‘erased’ (in certain circumstances); and
• To lodge a complaint with the Information Commissioner’s Office (ICO) who is our independent regulator for data protection.
